Labour leader Ed Miliband is to admit his party's leadership lost touch both with its own members and the public.

In a speech in Wrexham to his national policy forum, he will propose reforms aimed at making the party less insular and its decision-making more open.

He will say the last Labour government did not listen to concerns over issues like immigration and housing benefits.

The Tories say Labour ignores the way its leader is elected. They criticise the trade unions' role in that process.

In his speech to party activists and trade unionists, Mr Miliband is expected to unveil a series of proposals.

Its annual conference will be opened up to campaign groups and charities - who will be allowed to speak from the floor in debates although they will not get to vote on policies.

Local Labour parties will be allowed to organise petitions on issues which they feel ought to be debated at national level, and if they can demonstrate enough support, those issues will be placed on the agenda at future meetings of the policy forum.

There will also be a new code of conduct for the party's candidates and politicians to ensure they meet voters regularly and are transparent about their expenses.

And following on from an idea trialled in Oxford and Birmingham, local parties across the UK will get extra resources if they sign up more supporters.

Mr Miliband is also expected to cite the decision under Prime Minister Gordon Brown to abolish the 10p starting rate of income tax, which saw millions of low earners lose out.

In his speech Mr Miliband is expected to say: "'Old Labour forgot about the public. New Labour forgot about the party.

“The party was trying to tell us what the people wanted us to know ”

"And, by the time we left office, we had lost touch with both.'

Turning to the policies of the last Labour government, he will say: "We went from six people making decisions in a smoke-filled committee room to six people making the decisions from a sofa in Whitehall. Sometimes less than six.

"But the party was trying to tell us what the people wanted us to know. They were telling us about immigration, about housing benefits and about the 10p tax. We didn't listen."

He will go on to defend plans to scrap elections to the shadow cabinet. The move to take sole responsibility for frontbench appointments has been criticised by some backbench MPs who feel they will be distanced from the party's top team.

Mr Miliband will say shadow cabinet elections sees the party "look inwards not outwards".

"I want us to be an alternative government," he will say. "The only election members of the shadow cabinet should be worrying about is the general election."

BBC political correspondent Iain Watson said it is understood that Mr Miliband does not intend to address the party's leadership rules highlighted by the Conservatives, which allowed him to be elected with the help of union votes, despite more party members favouring his brother, David.

He has already said these will be revised as part of a series of changes which will go to the party's autumn conference, under the banner Refounding Labour.

Ouch: Patients Suffering From Docs' Poor Pain Management

Cindy Steinberg will never forget the day in March 1995 when chronic pain entered her life.

An accident at work left her crushed under a large file cabinet, resulting in torn ligaments and nerve damage in her back.

?I went from doctor to doctor,? Steinberg said. ?I tried nerve blocks, various injections, physical therapy and acupuncture.?

Still, she couldn?t get relief from the terrible pain. After long days of work as a manager of educational media and technology development, she often went home crying.

?I was lying on the floor during meetings,? said Steinberg, who lives in Lexington, Mass. ?Finally, after about years years, I found an osteopath who told me I had to give up my career.?

Steinberg is one of 40 million Americans suffering from chronic pain, and new research has found that the overwhelming majority of people like her are suffering from poor pain management. in fact, a series of new studies found that the medical community isn?t doing enough to manage different types of pain, including chronic, post-surgical and cancer-related pain.

Each of the three studies, published in The Lancet medical journal, reviewed treatment options currently available for each kind of pain.

The authors of the study on post-surgical pain said that pain management is improving thanks to regional pain relief, such as epidural analgesias and peripheral nerve catheters. For reasons that are unclear, however, patients are still suffering.

?Despite the introduction of new standards, guidelines, and educational efforts, data from around the world suggest that postoperative pain continues to be managed inadequately,? wrote Drs. Christopher Wu and Srinivasa Raja of the Johns Hopkins School of Medicine in Baltimore, the study?s authors.

They believe there could be a number of reasons, including a lack of pain assessment after surgery and doctors who simply don?t use every available method.

Dr. Patricia Baumann, assistant professor of anesthesiology at the Emory University School of Medicine, said epidurals and other types of regional anesthesia ? or anesthesia administered to a certain part of the body ? are very effective with many of the patients she sees, providing pain relief and preventing the need for opioids such as OxyContin.

?It blows my mind that it?s not better utilized,? she said. ?Sometimes, patients are afraid of epidurals, or the surgeons can?t stop long enough to have the epidural placed.?

Wu and Raja added that using multiple pain relief techniques at the same time ?may lead to substantial gains in the treatment of acute postoperative pain and potential reduction in the development of [chronic pain].?

Nipple dimpling? A sign of breast cancer?I?m 25. It just came to my attention that it seems my left nipple looks somewhat ?.?saggy? (for lack of better words). Like the skin nearly it. And when I raise my arms it sinks in a bit on the under half of it.
It kind of looks like..a ?U? shape on the areola right under the nipple.

I?m not sure if this would be considered ?dimpling? and a sign of breast cancer.

Calcium deposits on mammogram?I have a lump in my breast which my doctor thinks is a blocked milk duct. I had a mammogram done & a breast ultrasound this week so I don?t have the consequences yet. I had the mammogram first & then the next day returned for the ultrasound. They told me they sought after to redo 2 of the mammogram pictures since there were calcium deposits in the pictures. They reckon it?s cause I was wearing deoderant (I?ve never had one so I didn?t know I wasn?t supposed to wear deoderant). Back in 2004 I had my first mamogram at age 43. I didn?t take it honest in that there was NOBODY in my family that cancer.

They did the mamogram and saw a touch suspicious and called me back to get a further one that was a modest more wide. There were calcium deposits all over the place and they sought after to take out them.

I dredge up it being nearly Prayer and the Dr. doing the bioposy told me that there is a 30% chance that it?s cancer. Well, me thought that it would have to be impossible for anything to go incorrect, I knew I?d come out of this okay.

I despise to tell you this part but the bioposy was very awkward. There was a nurse there just to hold my hand and keep my cool. I never weep since of pain but this time I did. It twisted out that they indifferent 29 of these deposits. They check it out and I was cancer free but the terrible thing is since it was so awkward, I stanch I didn?t want to go for a further mamogram again which was really stupid. I felt that just being paid my yearly physical was enough for me. I wasn?t even doing self exams.

3 years later after bringing up the rear a bit of consequence, I was told by co-workers that it looked like my breasts were being paid less vital. We had a excellent laugh but the next daylight when I woke up and looked in the mirror I plotting ?yeah, they are being paid less vital and what is this here??

Under my arm looked a bit weird. Time to go get that mamogram I kept putting off! In mission up the Dr. to get a medical appointment for one, I was told that since I found a touch I had to see a Dr. In doing a examination he found a lump. Not the one that I originally had seen which he said was nothing but a lump near my nipple that I had never knew was there.

Immediately I was referred to a general practitioner. I?m not going to go into any longer detail about that but needless to say, when a further mamogram was done those calcium deposit were there (doubtless growing all along) and the lump was cancerous and there were all these masses.

I had to have a mastectomy on my left breast diagosed eventually with third stage cancer. I even had to have my nodes indifferent.

I can?t say for you that yours will be cancer. I don?t even know if the calcium had anything to do with it but I reckon it tells you a touch. Deal with the pain of being paid them taken out and make sure you have that mamogram done every year along with monthly self exams.

I kind of feel that in neglecting those things, that?s why I had cancer.

Oh, and I stopped wearing anti-persperant too.

I wish that there was a way that I could keep in supporter with you to see how all went and be support for you if you had any longer questions.

I am sorry that Shruken Fro had such a terrible encounter, but there are times I wish people people would try to refrain from scaring the crap out of other people.

First off, very nearly every single woman has at least one calcification which can be seen on a mammogram. By far, the margin of those are compassionate. There are many things, other than cancer, that can cause microcalcifications?..

?Microcalcifications will show up on a mammogram as tiny white flecks in the bandanna. Many times these are not cancer; but, in some instances they are associated with very early cancer ( ductal carcimona in situ). Consequently, they need to be investigated thoroughly.?

So, look at it this way?.if these do turn out to be malignant, you have found them very early! They have been found previous to a cancer has grown large enough for you to feel. The before a cancer is found, the better! And ductal carcinoma in situ has a VERY high cure rate when found early.

Secondly, a needle core biopsy must not be awkward. I have never heard of someone having 27 (or was it 29?) cores indifferent. The most I have seen taken are 10, and that was approaching it. A core biopsy, done either by mammographic guidance (in the case of fastidious masses or microcalcifications) or ultrasound guidance are really very simple procedures. And they are highly accurate. Since the radiologist can see the area of interest (mass or calcification), he can get a very accurate try out of that exact area. In the case of ultrasound guided core biopsies, we can really see the needle enter the mass or butt up hostile to the calcification, so we know the samples are accurate. i am suicidal?and well what i despise when people do is have pity..so thats not excellent..dont take it profesional..they will reckon that u reckon they are crazy..and well to be trustworthy ? just help em out physically?they will see how much u care abou them. they wont do it..just help em out physically,..
